Repeated Pregnancy Losses Overcome With Laparoscopic Surgery, Healthy Baby Delivered at KIMS Cuddles

The patient had previously lost her first pregnancy at 16 weeks and her second pregnancy at 18 weeks. During the second pregnancy, doctors had performed a conventional vaginal McDonald cerclage to support the cervix. However, the procedure did not prevent the pregnancy loss.
During her third pregnancy, doctors opted for a laparoscopic trans-abdominal cerclage (TAC) at 14 weeks of gestation.
The procedure was performed by Dr N. Bindu Priya, Senior Consultant and Lead Urogynaecologist, along with Dr Sindhura and Dr Sai Snehitha at KIMS Cuddles, Secunderabad.
Explaining the medical condition, Dr Bindu Priya said that some women experience cervical insufficiency, in which the cervix is unable to remain closed as pregnancy progresses. The condition can cause the cervix to open prematurely, sometimes without significant pain or contractions, leading to miscarriage or extremely preterm delivery during the second trimester.
“Given the history of recurrent second-trimester pregnancy losses and the failure of a previous vaginal cerclage, we opted for a laparoscopic trans-abdominal cerclage at 14 weeks in this pregnancy,” Dr Bindu Priya said.
In the procedure, a supportive stitch was placed at the junction of the uterus and cervix to provide stronger support and help prevent premature opening of the cervix.
Unlike conventional open surgery, the laparoscopic technique involves small incisions through which specialised instruments are used to perform the procedure. The suture placed around the cervico-uterine junction acts as a strong supportive band throughout the pregnancy.

Following the surgery, the patient was closely monitored by the medical team. She successfully completed her pregnancy and delivered a healthy baby boy.
Dr Bindu Priya said trans-abdominal cerclage may be considered in carefully selected patients, particularly women with a history of repeated second-trimester pregnancy losses and those in whom a previous vaginal cerclage has failed.
She advised women who have experienced recurrent pregnancy losses, especially during the third or fourth month of pregnancy and beyond, to seek specialist consultation early in subsequent pregnancies for appropriate evaluation, treatment and monitoring.
The doctors emphasised that the choice of treatment and the timing of intervention should be individualised based on the patient’s previous pregnancy history, cervical condition and overall clinical assessment.
